Completing your paperwork before you arrive at our office will help to speed up the process. Print out the 3 new patient forms and bring the completed forms to your visit. If you would rather complete the forms at the time of your visit, please arrive 15 minutes prior to your appointment time.

Personal Identification
A driver’s license or identification (ID) card is required.

Health Insurance Card
Please bring your health insurance card so that we may correctly bill your insurance.

Relevant Medical Records
Gather and bring any important medical records and laboratory test reports (x-rays, scans, etc) from other physicians, hospitals, or healthcare providers.

List of Questions
Write out and bring a list of questions and concerns. This list will help you remember everything you wish to discuss at your visit.

At Pain Specialists of Iowa, we understand the difficulties of the insurance navigation process. Our staff will help guide you through the process and put you at ease.
We accept most major insurance plans. However, it is still a good idea to contact your insurer prior to your appointment to verify coverage.

If you have any questions about insurance or your bill, please contact our billing department by phone. Their hours of operation are 8:00 AM to 4:00 PM, Monday through Friday.
